Trying fill guide form keeps asking

Am trying to fill in Guide price form and it keeps asking me for a title. I’ve put Mr before my name and that didn’t work.
That's very strange. I have just had a look at the form and the title is the box is immediately above your name. It is a drop box offering you the different options. If you still having problems please email me mark.lane@britannialanes.co.uk your name, email address, which county you are moving from and to, if you want a packing service, storage and if either of the propertied have a restricted access. I will then enter the information for you. Our removal price guide will email you the results. I hope this is of help regards Mark Lane.
